Case Management
We provide Specialist Case Management Services to those affected from brain injury and complex care needs.
Our Case Managers have medical background and many years of experience in brain injury flied.
We provide specialist advice and support to our Clients and their Families, to maximise their potential and help them improve their quality of life.
Each Client has their own Case Manager who will support them to prepare their individual Care and Support Plans.
We can provide individual and initial assessments of Client's needs, identify their rehabilitation needs and prepare a suitable rehabilitation programme that will help to improve their day to day quality of life.
Our Case Management Services can be provided in English and Polish language - to support cases where English is not Client's first language.
We can also offer packages of Neurorehabilitation in Poland with access to a range of Medical Specialists (Neurology, Physiotherapy, Neuropsychology and more) this includes referrals for specialist medical tests (MRI, CT scans).
General Management
We specialise in general and line management of domiciliary care teams. We have a lot of experience working with teams supporting client's with Traumatic Brain Injury, physical disabilities and other.
We offer support in 'start ups' programme - recruitment of new Staff, initial assessment and creating a care package that meets the Client's needs. Our programme includes bespoke induction training, related to the Client and their needs.
We provide on-going support to the Support Team, help in writing Care Plans and Risk Management Plans.
Our Case Managers or Support Managers will oversee the team, carry out regular supervisions and audits, to ensure Client's safety and wellbeing, but also to promote Staff personal development and professionalism at work
As Training and Management Specialists we believe education is the key to provide healthy and safe working environment, provide better understanding of Client's needs, and allow the Staff to professionally develop.
We offer bespoke Training Programmes to the Client, their Families and Care Teams.
Risk Management Plans
One of the most important parts of our job is to ensure Client's and Staff safety.
Therefore we carry out a specific risk assessments to provide professional advise on how to remove or minimise the risk.
We provide risk management plans that can be used by the Support Team in their day to day practice.
Disabled Friendly Holidays
Our Staff have a lot of experience in taking Client's on holiday.

We can provide professional advice on plannig such events, provide risk assessment plans and general support to the Client and the Team.
In as much as holiday usually brings good memories, for someone with brain injury this may be very stressful experience. We can provide advice how to overcome the fear of 'leaving the comfort zone' and allow the Client to enjoy themselves in a new environment.
Professional Services
We co-work with other professionals, who can offer their support and promote Client's independence.
Occupational Therapy services - we work with independent neuro-OT's who can provide adequate training and advice to the Client and their teams. They can help with planning daily activities and advise on what tools to use to make complicated tasks easy and achievable, promoting Client's independence at the same time.
Physiotherapy - we work with independent HCPC registered neuro-physiotherapists who can provide a specialist therapy packages following individual assessment of Client's needs.
